IT IS HIGHLY RECOMMENDED THAT YOU KEEP A COPY OF ANY MEDICAL PAPERWORK YOU SEND TO SCHOOL WITH YOUR CHILD

Please check with your healthcare provider to be certain your child has ALL required vaccinations.  Students may be denied entry if they are not in compliance with state laws regarding immunizations.  

All students must have proof of all required immunizations, or valid exemptions, in order to attend the first day of school. Documentation of immunity by confirming laboratory test results is acceptable.

Please note that in addition to the vaccinations required at entry to the early grades, THERE IS ALSO AN ADDITIONAL VACCINATION (the TDAP) REQUIRED PRIOR TO ENTRY INTO 7th GRADE.
